This single form covers everything: designating Bonaventure as the family's personal representative, selecting the scattering destination, authorizing the service, and confirming immediate need or preneed status. One form, one signature, complete enrollment.
This form would normally be incorporated directly into the checkout process within your platform. Families complete it as part of purchasing the service.

Instructions for Trustee or Executor
A formal letter to the family's trustee, executor, or personal representative explaining that the scattering has been pre-arranged and paid in full. Includes step-by-step instructions for activating the service when the time comes.

Pre-Planned Ash Scattering Certificate
A formal certificate confirming the service has been paid in full and the destination is reserved. Families are encouraged to keep this with their estate planning, trust, or advance directive documents.

Delivered to family after the ceremony
Completion Certificate
Certificate of Completed Ash Scattering
Formal confirmation that the scattering has been completed at the chosen destination. Includes GPS coordinates, the date of ceremony, destination name, and the family's unique service ID number.

Living Memorial Certificate
Confirmation of the donation made in the loved one's name to the USDA Forest Service Plant-A-Tree Program. Native trees planted in the national forest nearest the scattering destination.
